Social Prescriber - Job Description

The Social Prescriber will work as part of the practice team, the main role is to support patients to access all community opportunities available and to link people with community and volunteering support, so supporting them to take charge of their own health and well-being.

Duties

  1. Undertake assessments with referred patients to gain an understanding of their life, health condition/s and how it affects them, and to explore options for social support locally.
  2. To receive and action referrals from the practice team.
  3. Connect patients to local support and services supporting them to make contact. 
  4. Undertake follow up conversations to ensure that patients have been able to access the services. 
  5. Be proactive in seeking out new community support and activities for patients to access.
  6. Attend occasional Patient Participation Group (PPG) meetings as needed.
  7. Attend and support the Carers Group which has been established for some time.
  8. To encourage people; their families and their carers to provide peer support and to do things together, such as volunteering, Benson nature group.
  9. Participate in developing/attending meeting which promote social prescribing and health and wellbeing of the local community.
  10. Work with the PPG on local community projects.
  11. Liaise with the school on appropriate projects.
  12. Liaise with the Mill Stream Day Centre on appropriate projects.
  13. Be prepared to work flexibly and during evenings and weekend if required.
  14. Work closely with the reception, administration and Care Navigation Teams
  15. Work closely with the nursing team and doctors.
  16. Other tasks suitable for to this role.

Job responsibilities

  1. Following the practice safeguarding process where appropriate.
  2. Assist and support volunteers with small grant applications. 
  3. Assist and support patients with benefit applications and/or signpost them to other organisations who can provide that help/support.
  4. Ensure that all activities and work is monitored and can be evaluated.
  5. Ensure that social prescribing codes are input into EMIS 
  6. Adhere to data protection and data sharing within the PCN.
  7. Attend practice/PCN/local meetings as appropriate.
  8. Work within the practice Health and Safety Policy, ensuring that all practices and procedures are undertaken in accordance with the guidelines.

Medical scanner and coder administrator - Job Description

MEDICAL SCANNER AND CODER

Flexibility can be arranged – school hours can be discussed/accommodated.

Job Summary 

The scanner/coder works in the scanning/coding team.  The main role of the scanning and coding team is the administration of the clinical paperwork workflow – patient discharge summaries, hospital clinic letters etc so that the GPs only see the items they need to action, and the rest of the information is on the patient’s medical record ready for the next interaction.  This involves following a protocol that is safe & effective.  Summarizing of medical records is also an important part of the role. 

To be successful in this role you will enjoy lots of administration, data entry and have a good understanding of the implications of what you are coding, filing, or processing.  You will develop a broad understanding of General Practice and knowledge of medical terminology/clinical coding and scanning coding process.  You will receive on the job training with external training where appropriate. 

Duties

Responsibilities 

  1. Review all incoming correspondence into Mill Stream regularly throughout the day to process/forward according to the protocol.
  2. Check NHS 111 reports and follow protocol.Follow up with Duty Doctor if required.
  3. Review and allocate E-consults to the relevant person.Following the protocol.
  4. Check OOH reports and follow protocol.  Follow up with Duty Doctor if required.
  5. Scan and code letters and relevant correspondence into the patient medical records and forward to the appropriate person as per protocol.
  6. File electronic patient related documents onto the patient medical record using the codes agreed as per protocol.
  7. Raising appropriate requests on ICE (system for pathology requests) where appropriate.
  8. Return any incoming letters to the sender should they be sent to us incorrectly.
  9. Enter coded information accurately and consistently as per protocol.
  10. Distribute electronic results to clinicians as per protocol.
  11. Process and distribute incoming paper mail as per protocol.
  12. Scan documents for the team as required.
  13. Send relevant EMIS tasks to the clinicians as required.
  14. Identify relevant clinical information contained in patient correspondence and code as per protocol.
  15. Monitor the electronic in box for correspondence from other sources, such as Pharmacy First and Consultant Connect and process the documentation as per protocol.
  16. Summarising of medical records onto the patient electronic medical record.  Usually this is the medical records which come into the practice when a patient registers.
  17. Participate in the call and re-call scheme for appropriate patients.

Administration/Ad hoc duties 

  1. Support the practice with covid/flu vaccination programmes.
  2. Any other duties the management team feel fits with the scanning & coding team
