New Patient Registration
You are welcome to register with the practice if you live within the practice area. The surgery covers the area of Benson, Rokemarsh, Roke, Berrick Salome, Ewelme, Brightwell Upperton, Brightwell Baldwin, Britwell Salome, Dorchester-on-Thames, Shillingford, Warborough, Preston Crowmarsh, Crowmarsh Gifford, Oakley Wood, Swyncombe, Mongewell and Wallingford.
We will ask you to complete a form allowing us to request your medical records from your previous GP surgery (see 'Patient Registration Forms' tab above for more information). You are welcome to select the doctor you would prefer to see and this will become your usual doctor.
Existing patients who move within the practice area (above) or to Berinsfield, Chalgrove, Cholsey, Hailey, Ipsden, Newington and Nuffield need not re-register at another practice when they move.

Continuity of Care
At Mill Stream Surgery we think it is important that you get to know one doctor. Research shows that if people see one doctor on a regular basis they get better care and have better health as a result. Now we know this isn’t always easy, doctors, like other people, have to have holidays, work part time etc. However as we think continuity is important we would really encourage you to get to know one doctor and to try to see that particular doctor. So while patients are registered as a patient with the practice, they will also be allocated a usual doctor within our electronic records system to begin with. As patients get to know the surgery and the doctors, they may wish to change this to reflect the doctor you see the most, at which point you can simply speak to our reception team, who will be able to help you with this. At short notice if you have acute problems you may need to be seen by another doctor if your usual doctor is not available.
